Wrap form input elements in label tags

This patch puts <label> </label> tags around form input elements. In general this means that you can click anywhere on the text label for an element to activate the elements. In some cases this does not change behavior much; e.g. clicking on "Start date:" simply focuses the corresponding textbox. But it makes a big difference for checkboxes because you can click anywhere on the text to toggle the checkbox.

There were already label elements in place for the "Start date:" and "End date:" fields, but they did not encompass the input element, so I don't think they were doing anything before.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
