Support multiple front/url pairs for meek

See tpo/applications/tor-browser#42436 for details. We have a similar feature in Snowflake but I think we want to do something slightly different here, and support multiple front/url pairs rather than just a single URL (which corresponds to only one cloud provider) and multiple fronts.
