Scaling dynamic bridges

As @irl is working on the Bypass Censorship dashboard (tpo/anti-censorship/team#66 (closed)), we should:

• Look into the budget for new relay/bridge associations
• Define how to send bridgelines to the vantage point, so they can rotate the bridge when it's blocked
• Find other operators
