Think about cases for when to use the MiddleOnly flag

With 0.4.7.2-alpha (and 0.4.7.7 getting deployed on our directory authorities) we get the option to have relays in a MiddleOnly position according to the specification. We should come up with some ideas when we put relays into that position and document that on our wiki. /cc @gus
