TPA-RFC-26: LimeSurvey upgrade

Our current LimeSurvey instance at https://survey.torproject.org is to be migrated to LimeSurvey 5.

TPA-RFC-26 was published and outlines the steps that should be taken by TPA and survey authors in this context.
