dev notes: Draft IPT algorithm

Having slept on it a couple of nights, this is what I have come up with for introduction point management for hidden services.

I have not looked at the C Tor implementation. This algorithm is from first principles and the spec. Probably there are important things C Tor does that I have overlooked, and mistakes I am making.

CC @nickm, and I'm going to request review from @dgoulet who I think is probably our best expert on this stuff.
