Follow-on for guard restriction discussion.

This is a follow-on for !182 (merged). I've tried to split up commits based on whether they are formatting, content-related, or editorial.

The final commit on this branch (f597ecda) reformulates the handling of RST_OUTDATED_MD_DIRSERVER so that we no longer need to describe a separate handling mechanism for "temporary" and "permanent" restrictions.
