To my chagrin, stream isolation itself is not fully integrated into
torspec; I've had to refer to the C tor documentation and to
proposal 171.

(A full discussion of stream isolation doesn't belong in
socks-extensions.md, since it is not SOCKS-specific.)

Notably, the section previously included a list of extensions
as well as a list of missing features from the RFCs.
This commit makes it _only_ list the missing features.

Specifically:
 - Stream isolation is an extension and so goes elsewhere.
 - username/password hasn't been missing since 2011.
 - Allowing empty username/password values is not forbidden by the
   spec.
 - Extended error codes are an extension.
