Ignore MiddleOnly flag for now

Georg Koppen requested to merge gk/doctor:bug_40035 into master

Not all of the authorities are voting on the MiddleOnly flag which is causing noise on the consensus-health mailing list. Moreover, it's not clear at this point that reporting differences in MiddleOnly assignment is useful.
