Enhanced spam blocking

This adds additional spam prevention by:

1. renaming 'reason' field to 'address'.
2. added a honeypot field 'name' for spambots which ignores account requests if filled.
3. updated example reason.

(Although some kind of Captcha would be better than honeypots as specifically designed bots can definitely break into the system.)
