switch from Incidant Command to Incident Lead and other fixes to RFC-91

there's a flurry of tweaks here following the TPA meeting held yesterday where we debated incident response procedures (team#40421 (closed)).

this includes:

  • change from "Commander" to "Lead"
  • add examples of past incidents to give a better idea of what rotation and tasks mean
  • document severity levels
  • clarify that roles can be "rotated" not just "delegated"
  • document that we want more documented incidents
