Add Snowflake survey to Reports

@antonela I've copied the new format for splitting the links by view/download as per @nah's last patch to the personas page: !61 (merged)

This is following a user report of Gitlab struggling to render complex PDFs: tpo/ux/research#31 (closed)

I figured we should adopt this format going forward on /reports too? Thanks!
