Fix Accordian Identifiers

This MR seeks to fix the issue described in #168 (closed). It changes the anchor format from #question to #topic_question A potential drawback is if anyone went to the trouble of linking to the anchor, those links would break. This does not affect the permalinks however!

It has been a while since I've contributed here so please let me know if I'm missing anything.
