Migrate new machine docs to new swap file policy

Following adoption of TPA-RFC-55: Swap file policy, we should implement the changes to our new-machines documentation and templates.

• new ganeti instance docs
• new physical machine docs
• fabric installer disk templates

@anarcat I pushed a commit on tsa-misc repo that adds a create_swapfile fabric task, and calls it during the installation process. I'm not sure I've done the right thing all around, so review welcome!

Saw that fly by, it looks good! I'm always a little uneasy about adding stuff to that mess, but I guess that's the way to do it.

Thanks. I'm pretty sure it might need some adjustment eventually but at least the basics are there.
